2025-2026 Registration

We are so excited for your children to join Sunnybeam for the 2025-2026 school year. Sunnybeam has an incredible year planned and we can’t wait to provide your children with a wonderful, educational and enriching experience. All of the registration forms in this packet must be completed for your child to begin Sunnybeam in the fall.

Please complete and return all of these forms no later than July 28, 2025. Your student(s) will not be able to start school

without a complete file. Your child’s file will need:

● Student Information Form

● Permission, Release and General Liability Form

● Health Information Form

● Student Emergency Contacts Form

● Immunization Records (Certificate of Immunization or Certificate of Exemption Form, see below)

● Medication Consent Form

● Online Tuition Form

● All About Your Child Form (recommended)

Please also send a family photo for the teachers to use in the classrooms.

Certificate of Immunization Status

Please contact your child’s physician for a copy of your child’s most recent immunization status and either complete a downloaded form from the Washington State Department of Health or include the form that your doctor’s office may be able to provide you. Please review the chart on the following page for the Washington State Department of Health’s current required immunizations to ensure your child is up to date on his/her vaccines. This form must be signed by a parent/guardian and included in your registration packet.

Certificate of Exemption

Sunnybeam School follows the Washington State Department of Health guideline for required immunizations for preschool aged children. Please review the chart on the following page. If your child does not meet these requirements please complete a Certificate of Exemption form. Should the Department of Health update their requirements for the 2025-2026 school year, we will send out an updated chart. This form must be signed by both parents and a physician and returned with your registration packet:
